Tesamorelin

Tesamorelin (Egrifta® / Egrifta SV®)

In simple words

Tesamorelin makes the pituitary gland release the body's own growth hormone. It is approved for reducing fat around the internal organs in people with HIV and is also being studied in fatty liver disease and memory decline.

What this molecule is

44 amino acids. Trans-3-hexenoic acid + GHRH(1-44)NH₂. An analog of growth hormone–releasing hormone. ~5136 Da.

Why it is studied

FDA-approved (2010) — HIV-associated lipodystrophy. Phase III (n=412): −15.2% visceral fat over 6 months. A unique mechanism: acts through the GH axis, not through GLP-1. Being studied in MASH and cognitive decline.

How it works

Pituitary GHRH receptor → pulsatile secretion of endogenous GH → ↑IGF-1 → activation of hormone-sensitive lipase (HSL) → ↑lipolysis of visceral fat + ↑hepatic β-oxidation. Physiological feedback is preserved (unlike exogenous GH).

Doses used in studies

Studies used 2 mg SC once daily, every day, without titration, for courses of 26–52 weeks. Per the FDA label — strictly 2 mg/day.

Combined with in studies

  • GLP-1 agonists — different targets: GH axis + incretin axis
  • Ipamorelin — dual GH stimulation — just not at the same time
  • BPC-157 — GI tract + GH axis

Level of evidence

Level A Human clinical trials. There are randomized controlled trials (RCTs), or the compound has been approved by a regulatory agency. The most reliable level. FDA-approved 2010. Many Phase III trials.

Important to know

Honest caveat: FDA approval only for HIV lipodystrophy. MASH and cognition — off-label Phase II. Arthralgia 13%, edema 6%. Contraindicated in malignant neoplasms.
